Residency in Cardiovascular and Thoracic Surgery at KEM Hospital, Mumbai.
KEM Hospital, Mumbai is a premier teaching hospital of India and is a referral tertiary care centre doing about 1000 open heart cases per year besides performing about 200 closed cases. As a resident I was involved in the preoperative, intraoperative as well as postoperative care of the patient. We performed surgeries under supervision and were also involved in the training of nurses, perfusionists and Physiotherapists.
I performed more than hundred Open Heart Surgeries including 55 ASD closures, 40 Mitral Valve Replacements, 15 Aortic Valve Replacements and 5 double Valve Replacements. Besides, I performed 10 closed mitral commisurotomy, 5 PDA ligation, 5 aortobifemoral bypass and >20 interposition grafts for acute vascular injuries. I performed or assisted >100 embolectomies.
I assisted in CABG’s both on as well as off pump, closure of VSD, correction of Tetralogy of Fallot, coarctatation of aorta, bidirectional Glenn shunt, correction of TAPVC and Rastelli procedure.
I also assisted in Surgery for Aortic Aneurysm and pulmonary thromboeembolectomy.
In addition, I did a study on status of pulmonary artery pressure following closure of VSD in children less than 2 years of age. I presented a research paper on the same at the Chennai IACTS meet in 2002 and also presented my dissertation in Mumbai University for MCh in 2003.
September 2005-July 2005
Senior Registrar in Cardiac Surgery at Apollo Hospital, Chennai
Apollo Hospital, Chennai performs about 1500 Cardiac Surgeries per year 80% of which are CABG’s mostly off pump.
I harvested >200 LIMA’s, >100 radial arteries besides harvesting saphenous vein grafts.
I performed >150 proximal anastomosis and 50 distal anastomosis
Besides, I carried out a retrospective study on the effect of Cardiopulmonary bypass on postoperative renal function in patients with non-dialysis dependent chronic renal failure and presented a poster on the same at the 2005 IACTS conference in Cochin.
I applied for and secured an advanced clinical fellowship at the prestigious Mayo Clinic, Rochester, MN, USA and moved there in August 2005.
August 2005-September 2006
Advanced Clinical Fellowship at Mayo Clinic, Rochester, MN, USA.
I started an advanced Cardiac Surgery Fellowship at Mayo Clinic. It was a wonderful feeling to go to a place where all cardiac surgery started. I was involved in the preoperative counseling as well as intraoperative and postoperative care of patients.
Mayo Clinic performs about 3000-3500 Cardiac cases per year and is a referral centre for some of the most difficult and complicated cases.
I performed about 35 CABG’s other than putting 200 patients on Cardiopulmonary Bypass and weaning off Bypass. I started doing redo sternotomy under Dr. Hartzell Schaff’s guidance at Mayo clinic.
Mayo Clinic was a fantastic place to learn because it has the most advanced surgical as well as monitoring technology and the most dedicated surgeons. They perform the full spectrum of Cardiac Surgery including using homograft, Cardiac Transplants as well as regular use of Ventricular assist device and ECMO.
Working with stalwarts like Dr. Schaff, Dr. Mullany and Dr. Daly was a wonderful experience.
I worked for 6weeks in Cardiac Transplant. There were 4 Transplant Surgeons namely Dr. Richard Daly, Dr. Thoralf Sundt II, Dr. Joseph Dearani and Dr.Christopher Mcregor. The department was doing about 30-35 Cardiac transplants including Pediatric Cardiac Transplant. I was involved in 5-6 Cardiac Transplants besides implantation and management of Left Ventricular Assist Device[ LVAD]and Extracorporeal Membrane Oxygenator[ECMO]
September 2006 & December 2008
Lecturer [Assistant Professor] in Cardiac Surgery at KEM Hospital, Mumbai
I came back from Mayo and started working as lecturer in the Department of Cardiovascular and Thoracic Surgery at KEM Hospital, Mumbai.
KEM hospital and Seth GS Medical College is one of the oldest medical institutes in India and has a history of many innovations including Cardiac Transplant in late 1960s.
Recently, the Cardiac Transplant program has been restarted and they have performed a successful Heart Transplant after a long interval in 2024.
I was working with Dr. Anil Patwardhan who is mainly doing pediatric work leaving most of the adult cardiac work to me.
Dr. Anil M Patwardhan had trained in USA with Dr. Alberto Pacifico and had a keen interest in pediatric cardiac surgery as well as in arrhythmia surgery. He was performing a lot of studies in modified maze procedure and hence I also got a chance to take a lead in that procedure. He had devised modifications so that we could perform modified maze at very minimal expense in government institute where funding could be a problem many times.
I have done about 200 CABG’s [40 beating heart],>150 mitral valve replacements,>75 aortic valve replacements,>50 double valve replacements, 15 midline redo mitral and 4 midline redo aortic valve replacements independently. Besides I have closed >100 ASDs, ligated10 PDAs, created BT Shunts and repaired RSOV.I have excised about 25 myxomas [both left and right atrial]. I am regularly doing MAZE procedure for chronic atrial fibrillation. I have helped the residents perform cardiac surgical cases like ASD and valve replacements. I was also involved in training residents and post MCh senior residents in different aspects of coronary surgery like harvesting mammary arteries, radial artery, venous grafts, as well as performing proximal as well as distal anastomoses.
January 2009-August 2009
Associate Professor, CVTS at KEM Hospital, Mumbai
I was promoted after only 2years as lecturer to the position of associate professor and I worked with Dr. N B Agrawal at KEM hospital, Mumbai.
I had the responsibility to run the unit along with my unit head and involved clinical as well as administrative responsibilities.
I was also contemplating getting into private practice and I was offered a position of senior consultant at Bombay Hospital, Indore which I accepted and moved to Indore.
September 2009-May 2012
Senior Consultant Cardiac Surgeon at Bombay Hospital, Indore
I decided to move to Indore as an independent cardiac surgeon and have been performing both adult and pediatric cardiac surgery at Bombay Hospital, Indore.
I was regularly doing off pump coronary artery bypass grafting, valve replacement surgery as well as closure of ASD, VSD, PDA, repair of ruptured sinus of valsalva.
I have done more than 200 CABG, 50 mitral valve replacement and 25 aortic valve replacements at Bombay Hospital, Indore.
I did about 10 emergency CABG’s for unstable left main disease as well as for PTCA related complications with good results.
Consultant Cardiac Surgeon in Mumbai [2013 onwards]
I moved back to Mumbai in 2013 and joined Bombay Hospital, Mumbai.
I was doing about 25cases per month. My work included primarily beating heart CABG along with valve surgeries and few pediatric cardiac surgeries.
I am attached to MGM hospital Vashi and Apollo hospital, Belapur in Navi Mumbai. Both hospitals have got transplant and mechanical assist device programs.
Both the above-mentioned hospitals are NABH as well as JCI accredited and provide high quality and evidence-based care.
I am performing around 200 surgeries every year since 2014 with good outcome. In the last 10 years, I have done more than 2000 cardiac surgeries with less than 2% mortality.
I am mostly doing beating heart CABG [including high risk left main disease, emergency CABG and CABG in patients with severe left ventricular dysfunction, CABG combined with valve replacements],valve replacements, and selected pediatric cardiac cases like ASD repair,VSD repair,PDA ligation, repair of coarctation of aorta.
I have developed a significant volume of work and a patient base in Navi Mumbai over the last 2-3 years. I have performed many emergency CABGs as well as cases like thrombosed prosthetic valve in emergency in the hospitals of navi Mumbai with good outcome.
Associate Professor in Cardiac Surgery [ September 2018 – June 2022]
I accepted the post of associate professor in cardiac surgery at MGM New Bombay Medical College. It is a deemed university with intake of 100 students every year for graduation and boasts of all subspecialities including cardiac surgery. The university also runs courses for nursing , dentistry ,physiotherapy and lot of other allied health professions including perfusion technology. MGM university also has postgraduation as well as super specialty courses in most of the subjects
I was operating twice a week there while continuing my work in the private setups elsewhere.
I was also the acting head of department and the program eventually wants to start a viable heart failure, MCS as well as heart and lung transplant program
I have overall experience of independently performing more than 2000 off pump Coronary artery bypass grafting along with other routine adult cardiac surgeries
1st July 2022-30th June 2023
Clinical Fellowship in Heart and Lung transplant and Mechanical circulatory support
I pursued an advanced clinical fellowship at University of Alberta, Edmonton, Canada.
It is a very busy center performing the maximum number of heartmate 3 implants in Canada besides running a very busy heart and lung transplant program. The faculty here were extremely committed to training and it was a wonderful experience .
I had an amazing experience of about 70-75 double lung transplants,30 adult heart transplants and 31 heartmate3 implants. There were also BIVAD implants, ECMO and centrimag temporary LVAD and RVAD implants
The fellowship also had a lot of hands on experience including ECMO cannulation, Heartmate 3 implant, heart procurement, lungs procurement, implanting lungs in the recipient as well as implanting donor heart in the recipient.
I was involved in almost all the donor organ procurements in that year. I was also involved in the majority of the heart and lung transplants either as a first assist or as operating surgeon.
I am confident in procuring organs including from DCD donors as well as performing implants. I was also allowed to perform heartmate 3 implants and I am fairly confident of performing those too.
I was also involved in almost all the centrimag/levitronix and biventricular assist device implants and a significant number of ECMO cannulation/initiation.
It was an excellent experience at Edmonton and I had a fantastic work experience.
I shared an excellent rapport with the staff surgeons there as well as the departments of cardiology, pulmonology and the artificial heart team. I did have a very good understanding with the ICU consultants, transplant coordinators as well as nursing and administration staff in the hospital. I believe that I was able to function very well as a team member.
We also had cardiac surgery residents from other institutes in Canada, rotating with us to learn the nuances of mechanical circulatory support as well as both retrieval and implant in thoracic transplant. I had the opportunity to teach them both theory as well as help them retrieve a few lungs.
I am still in touch with my colleagues as well as staff surgeons, pulmonologists and ICU staff and enjoy their wholehearted backing in all my endevours.
Senior Consultant Cardiac Surgeon, MCS and Heart-Lung Transplant KD Hospital, Ahmedabad [September 2023 to March 2024]
After obtaining hands on training in ECMO, Centrimag and LVAD Heartmate3 implant for 12 months in a busy center in Canada, I joined KD Hospital, Ahmedabad as consultant cardiac surgeon and specialist in ECMO/MCS and heart and lung transplant;
This is a new transplant program having done the first double lung transplant in March 2023. So far our center has performed 5 double lung transplant and 1 combined heart-lung transplant.
During this short tenure, I was mainly involved in the independent procurement of organs from NDD donors and helping in the implant and postoperative care
I had procured 3 lungs independently during these 5 months with good outcomes.
I also continued to independently perform adult cardiac surgery which mostly comprised of off pump coronary artery bypass
I wanted to move back to Mumbai eventually since I had practiced in this city as staff surgeon since 2013 and had a good referral base here. Besides, though Mumbai is a vibrant center for cardiac surgical work, still lags behind many parts of India in the field of MCS as well as thoracic organ transplants.
I was offered a position at Gleneagles hospital, Mumbai which is a part of the international health care group called IHH Healthcare.
The Mumbai center performs more than 100 liver transplants , 60 kidney transplants per year and is trying to develop their heart and lung transplant program.
Senior consultant CVTS and Heart, Lung Transplant and MCS surgeon at Gleneagles Hospital, Mumbai [March 2024 to November 2024]
Having spent most of my cardiac surgical training years in Mumbai and practiced in Mumbai for 10 years, I was happy to move back to Mumbai when I was offered a job in Gleneagles hospital, Mumbai.
Gleneagles or IHH Healthcare is a renowned international group with hospitals in many continents and are very keen to develop their heart and lung transplant as well MCS program.
I am working here since last 5 months and have performed many off pump coronary artery bypass grafting, including some fairly complex ones, successfully. We have also done valve replacements, TAVI and surgery for aortic dissection.
Simultaneously, we are also trying to build a team as well as list patients for both heart and lung transplants. Presently, the primary challenge is to find trained personnel for these programs as well as focus on increasing organ donation. We also grapple with issues of financing our transplants.
The institution is very supportive of such efforts to build up a good transplant team to lay the foundation of a successful thoracic transplant program in the future. Our unit in Chennai has got a more established thoracic transplant program and with efforts, we should be making good progress in the years to come. We are making good progress in that we are already getting more referrals for lung transplants and hopefully heart failure referrals should also improve in due course.
Director of Cardiac Surgery, Medanta Hospital, Indore, MP ,India
December 2024 to June 2025
I was offered a position and promoted to director of cardiac surgery at the Indore branch of Medanta hospital . It is a part of Medanta group which is based in gurugram where it has a 1000+bedded hospital
I work mainly with adult cardiac surgery with 80-90% of my work comprising of off pump coronary artery bypass grafting. I am doing around 40 cardiac surgeries per month with my team of anesthetist , 2 perfusionist and 2 physician assistants .
We have excellent outcomes so far and there are plans to diversify into the heart and lung transplant and mechanical circulatory program once we consolidate and build a more robust and bigger team. This hospital has a history of having a heart transplant program earlier in 2015-2016 and we are trying to revive that program.
Clinical Fellowship in Heart and Lung Transplant Surgery and Mechanical Circulatory support at Mayo Clinic, Rochester, Minnesota
July 2025 onwards till date
I started a fellowship in heart and lung transplant surgery and mechanical circulatory support.
The motive being to stay connected to high volume and high quality and high risk transplant work. There is also the intention to venture into transplant work while still trying to maintain a certain volume of adult cardiac surgical work which I have been doing independently in the last 18-19 years.
I am looking forward to working in an environment where there is high volume regular cardiac work along with complex aortic work, minimally invasive and robotic cardiac surgery.
It is my feeling that compared to our population with end stage heart and lung disease ,we are doing few transplants and mechanical circulatory support in our country and we might be on the cusp of a massive surge in the field of heart transplant and MCS akin to the surge in liver and kidney transplant.
I will try to be a vital cog in the wheel for development in these fields along with the existing team which is already doing such stellar work.
